Safety demo gone wrong

As a lesson in how not to do it, this health and safety instructor could not have given a clearer demonstration.

But sadly for Paul Cavanagh, toppling over into the garden of the house next door was not part of the exercise and he has now been suspended by his bosses.

They say Mr Cavanagh, of Gateshead, made 'fundamental errors' while teaching a group of employees at Morrison Facilities Services.

His blunder came to light after a member of his group filmed the accident and posted the video on YouTube a week ago, which has had more than 22,800 hits.

In the footage, Mr Cavanagh is seen working outside a semi-detached house in Gateshead instructing his group on how to prevent accidents on ladders.

He is then seen climbing up with no hard hat, but then is thrown one by a colleague.

He is also caught standing on a rung when he takes his hands off the ladder several times.

Mr Cavanagh is attached to a tether and when he comes off the ladder, he is seen to swing around to the other side, with his back against the wall.

But as he tries to clamber back onto the right side of the ladder, it slips and crashes sideways, with Mr Cavanagh still tied to it, falling over a fence and into a neighbouring garden.

He lands next to a pile of children's toys in a front garden and after his fall, he is seen sitting in shock as members of the group come to his aid.

One worker remarked: 'You should have put the side tethers on'. While another employee says: 'Well, that proves the system doesn't work'.

The ladder should have been secured to the wall to prevent the slip, but it is alleged that Mr Cavanagh did not follow company protocol and has since been suspended from his job as an investigation takes place.

His employer, Morrison, said in a statement: 'Having reviewed the video ourselves it appears that a number of fundamental errors were made by this individual, which resulted in the accident taking place.

'These errors suggests that a number of our standard health and safety protocols were not followed and as a consequence we had no option but to instigate a suspension pending a full internal investigation.
